As the following item(s) are company-owned, I undertake the responsibility to take care of the mentioned item(s) and shall be liable to replace them with the same or similar make/model and/or cost of repair if lost or damaged.

I understand that any download or software requests are to be forwarded to the IT department. I will not attempt to download or install any application/programs without the management and IT department approval.

By signing this Agreement, I acknowledge that I have received the office equipment described above in working order and conform to the terms and conditions for the use of said equipment.
